Ancient Roots of Physiotherapy

The idea of using movement, touch, and natural elements for healing dates back to antiquity.

  • In ancient Greece and Rome, hydrotherapy, exercise, and massage were widely practiced in baths and gymnasia.

  • These methods were part of daily culture and preventive health, combining physical activity with community well-being.

  • Early forms of physiotherapy can also be linked to traditions of bone setting, herbal remedies, and therapeutic touch found across civilizations.

Thus, long before physiotherapy became a profession, it existed as part of cultural practices for maintaining physical health.

The division within the Indian Association of Physiotherapists (IAP)—the largest professional body meant to represent us—has created confusion, weakened our collective voice, and demoralized the community. Instead of standing together for progress, senior leaders often appear to be engaged in conflicts driven by ego rather than vision.

This internal fracture has left the profession vulnerable. When leaders fight, it is the entire community—and ultimately the patients—who suffer.


Declining Student Interest

A telling sign of this crisis is the decline in admissions to physiotherapy colleges across India. In recent years, fewer students are choosing physiotherapy as a career. This downward trend reflects not a lack of value in physiotherapy itself, but a perception problem—students and families see the profession as undervalued, underpaid, and unrecognized.

If this continues, the pipeline of future physiotherapists will shrink, threatening the sustainability of the profession.


The Threat of Paramedical Status

Another pressing challenge is the constant pressure from organizations like the IMA and MCI, which continue to push physiotherapy toward a “paramedical” label rather than recognizing it as an independent healthcare profession.

In states such as Himachal Pradesh and Uttarakhand, physiotherapists have already been classified under paramedical services. The consequences are severe:

  • Wages are shockingly low, often below those of nursing staff.

  • Postgraduate physiotherapists are made to work under graduate allopathic doctors.

  • The respect and dignity of the profession are undermined.

If this trend spreads across the country, physiotherapy will lose its independent identity and status in the healthcare system.


The True Value of Physiotherapy

It is important to remember that physiotherapy is not only a scientific discipline—it carries a deeply human and even spiritual dimension. The healing touch of a physiotherapist reaches patients not only physically, but emotionally as well.

This is why physiotherapy has often been compared to the healing acts of saints like Sai Baba and Jesus Christ, who brought comfort through touch. The profession goes beyond exercises and modalities—it restores dignity, confidence, and quality of life. To reduce it to a paramedical support service is to misunderstand its true essence.


A History of Division

Unfortunately, physiotherapists in India have historically been divided—by region (North vs. South), by language, and by leadership. In a country where more than half of physiotherapists are not even registered with professional associations, it is no surprise that we have struggled to build unity, recognition, and political strength.

In 2010, the profession witnessed what some called a “revolution” on Facebook. Though it began virtually, it briefly energized the physiotherapy community. Sadly, that momentum was never sustained. Many leaders lost focus, choosing personal interests and survival over the collective good of the profession.


What Needs to Be Done

The situation may be grim, but it is not irreversible. Reviving physiotherapy in India requires collective participation and concrete action:

  1. Unity Within Associations

    • The first step is resolving divisions within IAP and creating a strong, unified leadership that prioritizes the profession over personal conflicts.

  2. Recognition as an Independent Profession

    • Advocacy efforts must continue to ensure physiotherapy is recognized as a core healthcare profession, not just a paramedical support service.

  3. Raising Awareness Among Students and Families

    • We must showcase the real value and scope of physiotherapy—both scientifically and economically—so more students see it as a respected and rewarding career.

  4. Better Wages and Working Conditions

    • Physiotherapists deserve fair pay and recognition, especially those with postgraduate qualifications. Policies must reflect this reality.

  5. Professional Integrity

    • Every physiotherapist must commit to ethical practice and continuous learning, to maintain the credibility of the profession in the eyes of patients and society.


A Call for Participation

Friends and colleagues, we cannot remain silent any longer. Silence is equal to acceptance. If we continue to watch passively, the day is not far when physiotherapy in India will lose its independent standing altogether.

Clearing the Confusion: What Physiotherapy Really Is

In today’s world, patients often come across multiple therapies, fitness systems, and wellness services. While variety in healthcare is a positive sign, it has also created a lot of confusion about what physiotherapy truly is.

Again and again, I meet people who mistake physiotherapy for other fields—sometimes even for massage, spa treatments, or gym workouts. While all of these may play some role in health and well-being, they are not the same as physiotherapy. This confusion is not only misleading but also risks the dignity of our profession and the safety of patients.


What Physiotherapy Is Often Confused With

Here are some common areas where people mix up physiotherapy with other systems:

  1. Occupational Therapy (OT)

    • Occupational therapy is a separate healthcare field that helps people regain independence in daily activities, especially after injuries or disabilities. It focuses on adapting environments and teaching skills, while physiotherapy emphasizes movement, function, and physical rehabilitation.

  2. Neurotherapy

    • Many people have heard this term for the first time and assume it is a part of physiotherapy. In reality, neurotherapy is an alternative system with different principles and limited scientific validation. It should not be confused with neurological physiotherapy, which is evidence-based rehabilitation for stroke, spinal cord injuries, and other neurological conditions.

  3. Fitness Gyms and Spas

    • Gyms and spas are excellent for general wellness, relaxation, and fitness. But physiotherapy is a medical science that requires assessment, diagnosis, and treatment planning by qualified professionals. A gym trainer or spa worker does not have the same expertise as a physiotherapist.

  4. Ayurvedic Massage Parlors

    • Ayurvedic massage has its own traditional value, but it is not physiotherapy. Physiotherapy is rooted in anatomy, physiology, biomechanics, and rehabilitation science. Patients often confuse the two because both involve hands-on treatment without medicines, but the goals and methods are very different.



  5. Acupressure

    • Acupressure is another therapy often mistaken for physiotherapy because both are non-pharmaceutical. However, acupressure is based on pressure points from traditional medicine, whereas physiotherapy uses structured, research-backed techniques such as mobilization, exercises, electrotherapy, and rehabilitation strategies.


The Bigger Problem: Underqualified Practitioners

Another serious issue is the presence of underqualified or untrained individuals who claim to be physiotherapists or “trained healers.” They set up clinics or centers, use the name of physiotherapy casually, and mislead patients.

This malpractice not only harms patients but also damages the credibility of our profession. When patients receive poor results—or worse, injuries—from such centers, they lose trust in physiotherapy altogether, even when it has nothing to do with a genuine physiotherapist.


Why Awareness Is Essential

If these misconceptions and malpractices continue unchecked, the day is not far when the true identity of physiotherapy will vanish from the mainstream. Patients will not know whom to trust, and physiotherapists will struggle to maintain recognition for their hard-earned knowledge and skills.

Therefore, awareness is the need of the hour. Patients must be educated to:

  • Identify qualified physiotherapists by their degrees (BPT, MPT, etc.) and registration with recognized associations (like IAP).

  • Understand that physiotherapy is not a machine, not a massage, and not a spa service.

  • Realize that physiotherapy requires personalized, evidence-based treatment, not one-size-fits-all approaches.


A Call to Action

As physiotherapists, we have a responsibility to protect the dignity of our profession. That means:

  • Spreading awareness in our communities.

  • Correcting misconceptions whenever we encounter them.

  • Reporting and discouraging unethical practices by unqualified individuals.

  • Practicing with integrity so patients can see the difference between genuine physiotherapy and imitations.


Conclusion

Physiotherapy is a noble profession built on science, compassion, and patient-centered care. It should never be confused with fitness centers, spas, or alternative therapies. While these systems may have their own place in health and wellness, they are not physiotherapy.
