PHYSIOS OR ENTREPRENEURS, WHY NOT BOTH?

PHYSIOS OR ENTREPRENEURS, WHY NOT BOTH?

1. Clinical judgment – The know how to build clinical judgment and the process for a business are identical to a great extent. When one learns from his/her mistakes, it is called experience. When he/she learns from experience, it is called clinical judgment.

2. Uncertainty – Physios are used to dealing with uncertainty. Like entrepreneurs, physios make decisions with incomplete information. Sometimes they have to do things based on their gut.

3. Problem-solving trait – Just like businessmen, Physios too have a bias to action. A patient’s history, physical exams and tests, all are a routine part of care, just means of solving the problem the patient is suffering from.

4. Pattern recognition – Physios are excellent at it. They basically do 3 things: Make decisions, communicate/educate, and do procedures.

5. Question, observe, connect and associate – These core entrepreneurial skills are what makes a physio. In The Innovators DNA, the noted core skills of innovators were: questioning, associating, connecting, experimenting and observing.

6. Research and Experimenting – Entrepreneurship is about researching about things extensively and then experimenting with new ideas. Physios do this every day with their patients making them a pro at it.

7. Core entrepreneurial roles – Physios know how to be technopreneurs, market perceivers, managers, and/or investors.

8. Patients or Customers, both need same things – Physios have access to patients and understand the clinical issues more than anyone else. Just like customers, patients also have needs and wants.
9. Assess risk and make the spot cost-benefit decisions – Every medical decision made by a physio is based on the results of risks versus the benefits.

10. Take difficult decisions – Physios have the courage to know when something won’t work or should be ended. Physios deal with many grave circumstances and do what’s best for the patients. The same thing is required by an entrepreneur as well.

Amaran Anshan- A ray of hope to the physiotherapy professional from THE INDIAN ASSOCIATION OF PHYSIOTHERAPISTS (IAP)

PRESS RELEASE FROM THE INDIAN ASSOCIATION OF PHYSIOTHERAPISTS IAP)

14 July, 2017

THE INDIAN ASSOCIATION OF PHYSIOTHERAPISTS (IAP) TO ORGANISE AMARAN ANASHAN ( INDEFINITE HUNGER STRIKE) AT JANTAR MANTAR, NEW DELHI FROM 16TH JULY,2017 FOR ESTABLISHMENT OF CENTRAL INDEPENDENT PHYSIOTHERAPY COUNCIL IN LINE WITH MEDICAL COUNCIL OF INDIA (MCI) AND DENTAL COUNCIL OF INDIA (DCI).

The Indian Association of Physiotherapists ( IAP) is organising Amaran Anashan Indefinite Hunger Strike) at Jantar Mantar, New Delhi from 16th July,2017 for establishment of central independent physiotherapy council .Nearly 4,000 Physiotherapists from across India are expected to participate in the event.

Physiotherapy is an extremely important healthcare profession in present era. Physiotherapy course started in India in 1953, after 64 years also there is no council to regulate the profession. At present in India there are 296 Colleges offering undergraduate (B.P.T) Physiotherapy program consisting of 4 ½ years and 116 Colleges offering Masters in Physiotherapy (M.P.T) with 2 years duration. PhD programme of 4 years duration in Physiotherapy is offered at 16 Colleges and 11 Universities in our country.

There are 65,000 qualified Physiotherapists practicing all over India and at present there are 35,000 physiotherapy students in India . Till now we have no Central Council for Physiotherapy to regulate the profession and look into professional issues.

Formation of a Central Physiotherapy council will do following good for society.

1.Establish proper standard of Physiotherapy profession, practice and education.

2. Regularize the system of registration, recognition, standard, quality of education in Physiotherapy.

3. Check the unprofessional practice/quackery in Physiotherapy by untrained professional.

4. Give Physiotherapy its due professional status in the field of Health Care Facility and Health Science.

5.The council would create better rehabilitation and health care delivery services for the people of India.

IAP has been submitting several memorandums to Ministers and Elected representatives but there is no progress in UPA or NDA governments. During the present NDA Govt we have organized three massive Satyagrahas so far ( Dandi on June 19th,2016; Jantar mantar, Delhi on December 4th,2017; Varanasi on March 26th,2017) and filed several petitions to PMO, Health Minister. After observing no progress in our legitimate demands the we are organizing amaran Anshan at jantar mantar, Delhi from July 16th,2017 onwards.

As per World Health Organisation Physiotherapists are defined independent healthcare professionals to assess , plan and treat. The definition is "Physiotherapists assess, plan and implement rehabilitative programs that improve or restore human motor functions, maximize movement ability, relieve pain syndromes, and treat or prevent physical challenges associated with injuries, diseases and other impairments. They apply a broad range of physical therapies and techniques such as movement, ultrasound, heating, laser and other techniques. They may develop and implement programmes for screening and prevention of common physical ailments and disorders".

Even in elite sports like cricket the fitness certificate of player is issued by a Physiotherapist. This itself is direct testimony for professional independence.

There are 2,38,619 research articles indexed in Pubmed and several scientific documents of Cochrane collaboration proving the efficacy of Physiotherapy treatments in various ailments of Joints, Muscles, Ligaments, Brain Stroke, Parkinson’ disease, Low back Pain, Neck Pain, Arthritis and sports professionals.

According to census 2001 the elderly population in rural area is 12.2 million, and 31.8 million in urban area. There is no facility of physiotherapy in rural area. There are total 90 million elderly persons in India, which as per 2011 census are expected to grow to 173 million by 2026. In year 2001, 7.4% of total population was elderly.

About 64 per thousand elderly person in rural and 55 per thousand in urban area suffer from one or more physical disabilities. Most common disability among the aged person was locomotor disability which shows a continued rise in the numbers suffering from it.

Factors pointing the need of Physiotherapy System in the health care delivery to the society

Bad posture, neck and back pain, static posture while working for long hours etc. put more stress on human body and are mainly responsible for decreased fitness. These further lead to obesity, diabetes, hypertension, respiratory, musculo-skeletal and coronary artery diseases which may cause even stroke during the productive years of human life. Cases or such diseases or life style and increasing use of certain technological appliances are also growing and the trend if not prevented may adversely affect the demographic advantage.

According to a report by the Harvard School of public health, the economic burden of ailments of this type for India will be close to US$6.2 trillion for the period of 2012-2030. As these diseases affect the person in the productive years they cause immense loss of work days as well put immense pressure on public health expenditure. This is further escalated by the side effects of the drugs and needs to incur further expanses is for their further treatment.

Preventing the above mentioned ailments through non pharmacological means i.e. Physiotherapeutic system, in urban as well as rural areas will be a boon for the national economy. There are reports pointing that a 12.5% reduction in Ischemic Heart Disease, for example, could lead to economic savings of US $25 billion per year over the period 2011-2025 as reported by Prakash Upadhyaya in his article "An Overview of the Burden of Non communicable diseases in India" Further, by the year 2030, this productivity loss is expected to be equivalent to 17.9 million years lost.

To tackle this situation it is, therefore, imperative to have an adequate number of trained and qualified Physiotherapists. With globally comparable standards of their courses they can be given the responsibility for prevention and identification of such diseases and also early management and prevention of productivity and economic losses.

As the role of the Physiotherapist is preeminent in society and there are so many colleges running under reputed Universities establishment of Central Physiotherapy Council is definitely important and need of the hour.

From IAP we are hopeful that with our efforts of Amaran Anshan we will draw the attention of Central Government to establish Central Physiotherapy Council for Physiotherapists at the earliest for the betterment of society at large.

We need maximum support of the Press and Media for the genuine protest of Physiotherapists to Establish Central Physiotherapy Council.

Kindly Circulate the information to Press and Media. This above information can be used by every Physiotherapist/Physio student for news material.

How to make successful arrangementsf for inspection of Health department at your clinic/facility?

Dear Physiotherapists,

Health department has started inspections of clinics as per directives of higher authorities in Maharashtra. Municipal corporation & municipal council's squads are doing inspections across state. Got many calls from our professionals.  Hence writing it. It's lengthy but read. Kindly follow following things.

 1) Coperate with the inspection team . They are doing their job. Don't argue with them.

2) Keep your Degree Certificate, Council Registration certificate, AADHAR card, Profe of Clinic ownership / Rent documents like 7/12 certificate / Rent agreement or equivalent certificate. KEEP 4 PHOTOCOPIES  of all documents. Also keep orignal degree , registration certificate.

Kindly note that ASSOCIATION MEMBERSHIP certificate is not valid document for practice in state. Don't argue with inspection team on it. They are legally correct in this regard.

 3) As per Bombay Nursing Home Act there is no need to register clinical establishment  if you are running a clinic with "Less than 2 BEDS". Hence issue of "Qualified Nurse" , Fire  Brigade NOC, dont get applicable here.

Kindly note that "Beds" here means IPD bed. Inform inspection team that its OPD & patients are not admitted here.

 4) Inform team that Physiotherapy is "Non-Pharmacological"  treament & medicines are not given. Hence need of registered Pharmacy practitioner is not applicable.

DON'T KEEP ANY MEDICINES in clinic.

5) if you are doing only consulting then there is no need to register with "Bio Medical Waste" agency. Inform them that Physiotherapy  procedures doesnt include removal of blood  / body fluid & There is no CONTACT with blood/ body fluid.  U can give affidavit of same if inspectors demands. Nothing harm in this. But make sure on what you are signing.

If u r practicing dry needling then registration with bio medical becomes legal.
If u have it then no issues. If u don't have then DON'T STOCK NEEDLE PACKETS IN CLINIC.

If u r Using cotton for wiping Electrode GEL then it can be considered as  biomedical waste. Politely inform them in details that it's not coming in contact with blood / body fluid & it's not used for invasive procedure.  If needed give DEMO of it.

To Avoid conflicts for Time being u can use TISSUE PAPERS (Paper Napkins) for cleaning electrodes.

Keep the bills of purchase of gel.

 6) if you dont have anything mentioned above then request  for time. It's possible that u will be allowed for it & action will not be taken.

7) few tips for Bed:
Don't use typical beds used in wards for admitting people.
Customized counches  with storage capacity / cupboards  can be used.

If bed has I.V. stand then remove it.

8. Keep format of bill / bill pad.

9. In the last, Kindly remember that We r "REGISTERED PHYSIOTHERAPY PRACTITIONER" offering physiotherapy services as per law of land of state. We are committed to laws of land & we are following all laws which are applicable to us. Hence be Relaxed, be polite & don't Argue  with inspection team.

Be Legal, Do legal....
Cheers to Physiotherapy.

How to educate physician nearby about physiotherapy

Happy New year 2017!
My New Year's resolution is to make the physician more aware about PT.
In our day to day practice we come across amazing facts to know, about the mentality and unawareness of physicians or allopaths. These things sometime seems very strange, funny and puzzling to all of us. Their are some physicians and allopaths who know the importance of physiotherapy/ physical therapy. Motive of this post is not to hurt any allopathic doctor. We love our doctors and they are a critical part of the health care team, but there are some things we hope physicians/allopathic doctors should know about physiotherapy/ physical therapy, and things we hope informs the patient about physical therapy.
8 Things We Wish Physicians Knew About Physical Therapy/Therapists

(1) It’s not just a sheet of exercises.

Many times patients tell us that their doctor “gave them some home exercises” to do for their condition. While that may work from time to time and people want “home solutions” nowadays, there is much more to physical therapy than exercises.
1. We do a thorough physical assessment and manual therapy and mobilizations to help things move smoother and easier.
2.Time and time again, therapeutic exercise, education, manual therapy, and pain management is the tried and true method to get people better.
3. Physical therapists are experts at that and can significantly help patients lead healthier, happier lives following this treatment plan.

(2) Modalities don’t do a whole lot

For some reason, there are many physicians that have grasped onto the idea that physical therapy is all about ultrasound, TENS units, hot/cold packs, and electrical stimulation.
While these things may help at times, the evidence supporting their use is scant at best.
Physical therapy is much more than passive treatments like these.
Sure, they can help someone in pain “take the edge off,” but ultimately they will need manual therapy, exercise, and education to maximize their outcome.

Physical therapy is an “active” process in the end

(3) PT clinics have different specialities

Physical therapy have different super specialities for different cases.
Physical therapy for sports injury can not be taken best treated at paediatric physical therapy clinic. the setup for different clinics are different the doctor providing physiotherapeutic treatment is also different in his qualifications like ortho, neuro, cardio paediatric, sports or geriatric and many more.

It’s not just who’s closest to work or home.  Like any business, there are different levels of quality.

Patients should be encouraged to look for a physical therapy clinic that suits their individual needs case to case.

It would really help if physicians took the time to get to know the physical therapists in the area and help the patient make an informed decision, not leave it to front office staff.

(4) Residual deformity and Limping is not normal
Many patients walk into our PT clinics limping and say, “Doc said I could get rid of the crutches!”  Unfortunately, that is a really bad idea.  The abnormal walking pattern not only changes the mechanics of walking, but also can delay healing, increase pain, prolong swelling, and delay return of function.  You should use an assistive device of some kind until gait is normalized.

(5) Patients need a few visits after any soft tissue or joint surgery
We’re always surprised how many patients tell us that “ My doc said I didn’t need therapy after this surgery.”
The problem with this is that many people unknowingly compensate after a surgery and this prolongs the healing process and think is is the amount of deformity we have to live with.
Most of people end up needing physical therapy more later than if they would have come right away.

Due to the delay the number of visits required increased some time to double.
That is obviously more time and treatment cost on the patient.

(6) Physical therapists are also source of new business to physician

Physical therapists are also trained to pick up “non-musculoskeletal problems.”
Many patients come to physical therapy complaining of things that are systemic in origin, not musculoskeletal.
Physical therapists are trained to recognize when a patient presents with symptoms that are not compatible with physical therapy and properly send them on to their doctor.
Having a good relationship with a physical therapist helps facilitate this with patients.

(7) Many problems patients have are in areas other than where the pain is.

For example, many patients complain of shoulder pain but the problem is really in the neck.  Another example is that many patients have low back pain and the patient gets MRI’s, injections, etc for the back but the problem lies in the hip.  If patients aren’t responding to treatment or if MRI’s are inconclusive, consider sending them onto a PT for a thorough biomechanical assessment to determine the source of the problem.

(8) Physical therapy is a better option than injections or pain medications.  Several studies have shown that physical therapy is better for chronic back pain, and for tendinopathies, injections are no better than therapy after 3 months.  We’re obviously a little biased but why not try the cheaper PT before an expensive MRI or try PT before a more invasive injection.  Sure, injections are needed at times to help calm the pain down so the patient can do PT – we have no problem with that.  However, injections aren’t a panacea for everything.
