Useless treatments in physical therapy

Electrotherapy is useless in low back pain

The Australian Physiotherapy Association recently declared that some  treatment interventions which are commonly used by physiotherapists/ physical therapists are useless and were no longer being supported.

You would be surprised to know that electrotherapy for lower back pain and repeated manual therapy for a “frozen shoulder” are among  physiotherapy treatments, the profession now believes these are a waste of time and money which is a lose for patients and therapist also. Details are available in APA web link.

Analysis of Dandi Satyagaraha

Dandi Satyagaraha for physiotherapists council of India organized here in Dandi. It was a good gathering of physiotherapists and students of physiotherapy. According to Facebook post of Dr. Umashankar Mohanty the event was a great success. More than 200 media stakeholders are said to be present for the coverage of the event, which includes both electronic and print.
He also added that, we are sure the media coverage and selfless efforts of  Physiotherapists/Physiotherapy students will fulfil our legitimate demand to form Independent Central Physiotherapy Council.

Where most of the physiotherapists are happy with this move some also have shown resistance towards the satyagraha they are saying that leader in this satyagraha are fail to make state councils at their respective states. And also ask to reunite the two partitions of IAP.

Think before you touch: Good touch, bad touch

Physiotherapists delivered their treatment thought skilled hand and sharp mind. Their hands are said to be magical hand.
Manual therapy is delivered to patients with magical results. Effective ness of the touch is dependent upon its type.

Types of touch:
Soft end feel
Hard end  feel.

1. Soft end feel: it is the end feel due to muscles and soft tissues .
Therapist can go little beyond the soft end feel during exercises.

2. Hard end feel : this is the end feel due to bone tissue. Therapist should not go beyond hard end feel.

Why end feel is important?

End feel is important in order to save damage to various body tissues and structures.

Psychology of touch:

it is very important to understand the psychology of touch. Intentions are closely observed by patients so, touch the patients by pure intentions.
Good touch and bad touch are easily recognised so pure intentions are good for healthy treatment.

Immunise your clinic from financial harms

Immunise your clinic from financial harms

Gone are the days when doctors are considered to be a God, in India daily you can see the news of attacks on doctors and hospitals by mobs. Patients and their relatives put all blame on doctors and hospitals.
Being a medical professional, your first duty is to take care of your patients, but the business person in you needs to think about keeping risks and losses to zero.

Protest after attacks on doctors and hospitals

Consider if there is a fire instance at clinic your patient records may get destroyed by fire, secondly your computer equipment stolen, third most important your medical equipment, modalities were to break down.
Can you carry your business further.
Are you safe from the financial repercussions and interruption of business?

It is a common trend in India to beat the doctor and distroy the healthcare setups by relatives  after any misshaping. Latest example is Noida where a troop distroy everything and harm the doctors.

So insurance of your healthcare setup is the only protection which can cover you wide range of risks

Even in the best-run hospitals, accidents do happen so don't let these accidents distroy the business you've worked so hard to build.

Risk free clinical practice

There are insurances which includes  coverages as well as several extended care options like giving you the flexibility to design a plan to suit your special needs and also protect you from the  loss.

Your insurance can include provisions for a wide variety of risks like:

1. Damage to business property including buildings, contents, medical equipment, and revenue loss due to work interruption arising from an insured loss.

2. Legal costs due to injury to a third party or damage to their property while on your setup.

3. Damage to computers and electronic media.

4. Loss or damage to valuable papers and records.

5. Equipment/ modalities breakdown.

6. Employee theft.

7. Losses related to identity theft.

8. Disease outbreak which may leads to temporarily relocation.

So every healthcare setup should be insured to provide a tension free care to patients.

How to use crutches

Crutches and how to do crutch walking

In present scenario where life is fast moving we came across multiple traumas most common injuries are traffic accident related and the body part most effected is leg or foot. Commonly crutches are advised in these injuries, so Crutches are supports that help you walk when you have an injured leg or foot.

How to use?

Walking with crutches:-

First, hold the grips of your crutches, then bring the crutches forward evenly, keeping your injured leg off the ground. Lean forward, putting your weight on the grips. Swing your good leg forward, placing your foot just in front of the crutches. Again repeat the same for next move.
Warning:- never rest your armpits on your crutches. Body weight pressure on the armpits can cause damage to nerve complex that pass through your armpits.
In some cases your doctor may allow you to put some weight on your injured leg while you are using crutches, but make sure you always follow your doctor.

How to getting up from a chair, wheelchair and bed:-
Good quality crutches from http://medicobola.yolasite.com

First of all never forgot to lock your wheel chair.
Use the hand on the side of your injured leg, hold both crutches together by the grips. Use your other hand to push up from the chair or bed same time you also push up on the crutches. Stand on your good leg. Make your balance and bring your crutches into position on each side before you start to walk. Wait for a while only after confidant standing you have to start walking.

Sitting down:-

Use the hand on the side of your injured leg, hold both crutches together by the grips. Hold onto the chair or bed with the other hand and lower yourself slowly.

Stairs:-

Going up stairs:-

Get close to the stairs. Step up with the good leg. Then bring the crutches and the injured leg up to the same step. Repeat for each step.

Going down stairs:-

First bring the crutches and the injured leg down to the lower step. Then step down with the good leg. Repeat for each step.

Always remember: "Up with the good, down with the bad."

Precautions:-

1. Be careful not to slip on water.

2. Your crutches may rub against the skin between your arms and chest. You may want to use good body lotion or talcum powder to prevent skin rashes.

3. If your hands get paining or tired, please put extra padding on the crutch grips.

4. If you feel continuous pressure on your armpits even using the crutches correctly, the crutches are too long and need to be shortened.

5. Always look for the grounds on which you are walking. Avoid obstacles if any.

6. Change your stick furale or base rubber on damage.

7. Select crutches according to your weight.

Always consult a rehabilitation specialist if you are using crutches, my personal favourite is a physical therapist also known as physiotherapist.
They are most eligible to prescribe and train you for crutches.

Logic or yogic

Dear physiotherapists brothers and sisters,

The Centre government 's move to include yoga in physiotherapy courses in universities is a dictatorship decision and government wants to imply this on physiotherapists of India who are already struggling for their existence with out a council.

As you all know Physiotherapy has no council on national level in India.

We should protest the yoga syllabus circulated by the UGC for inclusion in the curriculum for Bachelors of Physiotherapy (BPT) and Masters of Physiotherapy (MPT) courses to various universities

Under instruction from the Human Resource Department and AYUSH ministries, UGC secretary J.S. Sandhu on May 19 wrote to vice-chancellors of all universities to include the yoga syllabus under the BPT and MPT courses being offered by their departments and affiliated colleges.

We physiotherapists of India wants to know who was consulted before this dictatorship decision any physiotherapists or organisation representing physiotherapists was consulted or not.
Yoga have preventive qualities but on therapeutic values i am not sure if any have any evidence of that?

Physiotherapy is evidence-based knowledge that treats patients with specific interventions. If yoga is applied without proven benefits for specific problems, there is a possibility the patient's condition might worsened.

The HRD ministry had set up a panel headed by chancellor of Swami Vivekananda Yoga Anusandhan Sansthana, that drew up the syllabus. According to the syllabus, students pursuing BPT and MPT courses would learn and practise yoga postures. Physiotherapy students would be taught the yogic perspectives from the Bhagwad Gita, Ramayan, Mahabharat, Narada Bhakti Sutras and the Upanishads. They would also learn about Bhakti and Raja Yoga, benefits of Sanskrit chants, hymns, bhajan, satsang, mantra chanting and samadhi.

The syllabus says students would learn about the use of yoga for management of cancer, where it has been found to improve the quality of life of breast cancer patients receiving chemotherapy, gastrointestinal disorders, musculo-skeletal problems, cerebro-vascular events such as strokes, respiratory issues, cardiovascular disorders, and endocrinal and metabolic disorders.
Why we have to research on subjects other than physiotherapy we don't have any dearth of research topics in Physiotherapy.

UGC stakeholders must be logical before making such recommendations.

Firstly, therapeutic effect of yoga for specific problems should be proven where physiotherapy interventions also required.
What is the connection between the two?

One is logic other is yogic!😠
